THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

Your Rights

You have the right to:

• Get a copy of your health and clinical records. You can ask to see or get a copy of your record. We will provide a copy or summary, usually within 30 days of your request. We may charge a reasonable, cost-based fee.

• Ask us to correct your records. If you believe information in your record is incorrect or incomplete, you can ask us to correct it. We may say no, but we will tell you why in writing within 60 days.

• Request confidential communications. You can ask us to contact you in a specific way (for example, only by cell phone) or at a specific address. We will accommodate all reasonable requests.

• Ask us to limit what we use or share. You can ask us not to use or share certain health information for treatment, payment, or our operations. We are not required to agree — but if you pay for a service in full out of pocket, you can ask us not to share that information with your health insurer, and we will agree unless a law requires us to share it.

• Get a list of those with whom we have shared information. You can ask for an accounting of the times we have shared your health information in the six years prior to your request, who we shared it with, and why.

• Get a paper copy of this privacy notice at any time, even if you agreed to receive it electronically.

• Choose someone to act for you. If someone has medical power of attorney or is your legal guardian, that person can exercise your rights on your behalf.

• File a complaint if you feel your rights are violated. You can complain to us using the contact information below, or file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights, 200 Independence Avenue SW, Washington, D.C. 20201, by calling 1-877-696-6775, or online at www.hhs.gov/ocr/privacy/hipaa/complaints. We will never retaliate against you for filing a complaint.

Your Choices

You have both the right and the choice to tell us whether to share information with your family, close friends, or others involved in your care, or in a disaster relief situation. If you are not able to tell us your preference — for example, if you are unconscious — we may share your information if we believe it is in your best interest.

We never sell your information or share it for marketing purposes without your written authorization. Psychotherapy notes receive special protection: in most circumstances, we cannot use or disclose psychotherapy notes without your written authorization.

Our Uses and Disclosures

We typically use or share your health information in the following ways:

• To treat you. Your counselor may coordinate with other professionals treating you, with your consent where required.

• To run our practice. We use your information to operate the practice, improve your care, and contact you about appointments.

• To bill for your services. We share information with your health plan to obtain payment for sessions.

We are also allowed or required to share information in specific circumstances defined by law: public health and safety purposes (including reporting suspected abuse or neglect, or preventing a serious threat to anyone's health or safety); as required by law, including compliance reviews by the Department of Health and Human Services; responding to court or administrative orders and, in limited cases, subpoenas; limited law-enforcement and government requests; and workers' compensation claims.

Pennsylvania and New Jersey law provide additional protections for mental health records that in some cases are stricter than federal law. Where state law is more protective of your privacy, we follow state law.

Our Responsibilities

We are required by law to maintain the privacy and security of your protected health information. We will let you know promptly if a breach occurs that may have compromised your information. We must follow the practices described in this notice, and we will not use or share your information other than as described here unless you tell us we can in writing — and you may revoke that permission in writing at any time.

Changes to This Notice

We can change the terms of this notice, and changes will apply to all information we have about you. The current notice is available in our office, on our website, and upon request.

Website Privacy Policy​

This policy describes what this website collects from visitors and how we use it. It is separate from the Notice of Privacy Practices above, which covers the health information of clients in our care.

Information You Provide. When you submit our appointment request form, we collect what you enter — such as your name, phone number, and email address — so we can respond to your inquiry. This website operates with PHI Protection enabled under a Business Associate Agreement with our website platform, and form submissions are handled through HIPAA-compliant channels. Submitting a form does not create a counselor-client relationship. Please do not use this website for emergencies — if you are in crisis, call or text 988 (Suicide & Crisis Lifeline) or call 911.

Information Collected Automatically. Like most websites, we may collect basic technical information — such as browser type, device type, and pages visited — through cookies and similar technologies to understand how the site is used and to improve it. We do not sell or share visitor information with advertisers or data brokers.

How We Use Information. We use the information visitors provide to respond to inquiries, schedule appointments, and operate and improve the website. We do not use visitor information for marketing without consent.

Third-Party Links. Our site may link to third-party websites, such as insurance providers or professional directories. Their privacy practices are their own.

Children's Privacy. This website is not directed to children under 13, and we do not knowingly collect personal information from children under 13 online.

Your Choices. You may contact us at any time to ask what information we hold from your website inquiries or to request its correction or deletion, subject to legal record-keeping requirements. Most browsers also let you block or delete cookies.

Your Right to a Good Faith Estimate

Under the federal No Surprises Act, health care providers are required to give clients who do not have insurance, or who choose not to use insurance, an estimate of the expected cost of services.

You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency services, including psychotherapy. You can ask Six Ten Counseling for a Good Faith Estimate before you schedule a service, and we will provide one in writing at least one business day before your appointment. You may also ask for an estimate at any time during treatment.

If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

Make sure to save a copy or picture of your Good Faith Estimate.
